This program was introduced in 1985. The patient signed up by main and paid $135 and received a kit which included a history form and a vial which was taken to the PCP's office. The PCP's office drew the blood and the PCP submitted his History and Physical. The PCP was paid $35 for the consult.

This could be done online with a virtual visit. The blood could be drawn by AFL nurse managers at the clinics or outside PCPs.
